TERMS OF REFERENCE
HIW will carry out a limited and focussed review of the Trust’s incident and reporting arrangements in particularly in respect of the mental health directorate but having due regard to the wider context.
The review will examine arrangements for incident reporting, investigation of incidents, incident management.
The review will proceed by the way of a documents review and interviews with Trust staff.
HIW will report upon the findings of the review by spring 2008.
