Comments:	1.1 The Delivery and Support Unit (DSU) was invited by Paul Miller, Chief Executive of Velindre NHS Trust to undertake a diagnostic intervention within the Radiotherapy department at Velindre Hospital, Cardiff. 1.2 Velindre NHS Trust has expressed concerns that the achievement of the 31 and 62 day targets could be compromised where radiotherapy is the first definitive treatment for patients diagnosed with cancer. 1.3 The pathway for such patients usually involves a planning period and a staged treatment period. This causes challenges for scheduling, and a need for the entire patient journey to be appropriately coordinated. 1.4 Currently, the Radiotherapy department at Velindre Trust regularly monitors patients who receive their first treatment within 28 days of referral, as recommended by the Royal College of Radiologists. Performance against this target is variable. It has been perceived by staff in the department that this is due to a series of events involving the mechanical breakdowns of some ageing equipment and staff shortages. Currently the department is unable to chart performance against the 31 and 62 day wait targets, as data is not reliable across the healthcare community. 1.5 In March 2006, performance levels indicated that 19% of radical patients and 51% of palliative patients received their radiotherapy treatment within 28 days. Data from within the department has also shown that only 4% of radical breast cancer patients received treatment within 28 days. 1.6 A transitional plan for services at Velindre Trust has been shared with the DSU. A large component of the plan involves investment in both staff and equipment. This diagnostic report will further inform the transitional plan.
